1. From Father and Mother who are both Venezuelan by Birth

Requirements:

	Complete the Demographic Control Form, which can be downloaded from this link.
	Complete the Birth Registry Form, which can be downloaded from this link.
	Original Birth Certificate issued by the corresponding authorities in Hong Kong or Macao SAR, and three (3) photocopies.
	Original and two (2) photocopies of the three (3) first pages of the passports of the father and mother of the child.
	Original and two (2) photocopies of the marriage certificate of the parents.
	In case the father or the mother is a Venezuelan by birth, but has been born abroad, the respective birth certificate registered by the corresponding Venezuelan Consular Office should be attached.
	In case the parents cannot do the registry in person, it is necessary to grant a special authorisation duly notarised in Hong Kong or Venezuela).
	In case only of the parents can be present, he/she should bring an authorisation from the other parent, duly notarised in Hong Kong or Venezuela.
	The Original Birth Certificate must be accompanied by the respective APOSTILLA (certification of the signature of the official issuing the document).

2. From Father or Mother with Venezuelan Nationality by Birth

Requirements:

	Complete the Demographic Control Form, which can be downloaded from this link.
	Complete the Birth Registry Form, which can be downloaded from this link.
	Original Birth Certificate issued by the corresponding authorities in Hong Kong or Macao SAR, and three (3) photocopies.
	Original and two (2) photocopies of the three (3) first pages of the passports of the father and mother of the child.
	Original and two (2) photocopies of the marriage certificate of the parents.
	Declaration of Desire to become a Venezuelan citizen (document to be signed at the Consulate by the father or mother who is Venezuelan, in which expresses the desire that the child becomes a Venezuelan national in accordance with the Second Transitory Disposition of the Venezuelan Constitution).
	The representative (father or mother) who is a foreigner, must bring a duly notarised authorisation giving its consent to the registration of the child as a Venezuelan national.
	¡P The Original Birth Certificate must be accompanied by the respective APOSTILLA (certification of the signature of the official issuing the document).

3. From Father or Mother with Venezuelan Nationality by Naturalisation

Requirements:

	Complete the Demographic Control Form, which can be downloaded from this link.
	Complete the Birth Registry Form, which can be downloaded from this link.
	Original Birth Certificate issued by the corresponding authorities in Hong Kong or Macao SAR, and three (3) photocopies.
	Original and two (2) photocopies of the three (3) first pages of the passports of the parents of the child.
	Original and two (2) photocopies of the Gazette or Constancy by which the Venezuelan nationality of the father/mother was obtained.
	Original and two (2) photocopies of the marriage certificate of the parents.
	Declaration of Desire to become a Venezuelan citizen (document to be signed at the Consulate by the father or mother who is Venezuelan, in which expresses the desire that the child becomes a Venezuelan national in accordance with the Second Transitory Disposition of the Venezuelan Constitution).
