Death Registry:

	Complete the Death Registry Form
	Complete the Demographic Control Form.
	Original and four (4) certified copies of the Death Certificate issued by the corresponding authorities of Hong Kong or Macao SAR.
	Passport of the deceased.
	Two (2) photocopies of the three (3) first pages of the passport of the deceased.
	The Original Death Certificate must be accompanied by the respective APOSTILLA (certification of the signature of the official issuing the document).
	Bank receipt of the deposit of THIRTY US DOLLARS (US$30.00) in the account of the CONSULATE GENERAL OF VENEZUELA.

This Consular service requires three (3) working days.

REQUIREMENTS TO BE FULFILLED BY THE UNDERTAKER'S ESTABLISHMENTS

In case the body is to be transported to Venezuela:

	Authorisation of the relative of the deceased duly notarised.
	Death Certificate duly authenticated by the corresponding authorities and legalised by the Consulate.
	Proof of the adequate embalming of the body.
	Certified proof of the embalming of the body (issued by a pathologist).

In case of cremation:

	Authorisation of the relative of the deceased duly notarised.
	Death Certificate duly authenticated by the corresponding authorities and legalised by the Consulate.
	Letter of cremation.

The Ministry of Health and Social Development of Venezuela demands as a requirement for the transportation of bodies/ashes, proof that the country of origin and area where the death occurred, are free from contagious diseases.
