The applicant together with his parents, must come to the Consulate General to put his signature and fingerprints on the new passport.

	Complete the application form. The application form must be signed by both parents or legal authorized person
	Original and one (1) photocopy of the Venezuelan Identity Card or the birth certificate of the minor
	Six (6) photographs, taken recently, passport size
	Original and one (1) photocopy of the Venezuelan Identity Card of the mother and father of the minor or his legal authorized person.
	Bank receipt of the deposit of EIGHTY US DOLLARS (US$80.00) in the account of the CONSULATE GENERAL OF VENEZUELA.
	The minor must come accompanied by both parents. In case the parents are divorced, the mother or father must bring the original and photocopy of the document granting the non-shared custody of and the parental authority over the child. In case one of the parents is dead, the surviving parent must present a copy of the corresponding death certificate.
	In case one of the parents cannot be present, an authorisation signed before a notary granting permission to the other parent to request the passport for the minor.
	If none of the parents can be present, an authorisation signed by both parents before a notary granting permission to the relative or person with temporal custody over the child must be presented.
